This is a strange one indeed, but I"m not sure if there's a way to fix this short of a factory reset.
I had my new Xbox One S in 4k mode, and was playing around with the settings. I went to "advanced" and changed it from "auto-detect" to HDMI (instead of DVI).
It switched me to 640x480 mode, offering up to 1080p manual resolution select. So I switched it back to automatic. Except instead of going back to 4k, like it was before, it went to 1080p. I've done a full shutdown, several reboots, unplugged and replugged the HDMI cable, and it still only lets me pick between 720p and 1080p.
I just simply can't get it to let me select 4K as an option again.
Anyone run into this? When I first set it up, after doing the initial setup where I logged into Xbox Live, it said "your TV supports 4k, use it?" and I said yes. I had switched back and forth between 1080 and 4k a couple times to see if any of my TV's inputs supported the HDR stuff (none do, bah), but somehow going from Auto to HDMI to Auto broke it.
